FinalForms Information

A program called FinalForms is in effect for all North Posey sports.

This program enables all additional extracurricular permission forms (other than the actual IHSAA Physical Form for Athletes) to be completed online by parents and students.

Use the FinalForms links at the right to sign up for an account, to register your students, and to access the Parent Portal once you have an account.

NOTE: The current year IHSAA Physical Form must always be completed on hard copy. It can never be completed online.
